What Does the Package Rate Include?
The ECHS SOP April 2026 provides a detailed list of everything covered inside a package rate. All of the following are included —
Accommodation charges including patient diet, admission charges, anaesthesia charges, cost of medicines and consumables, cost of surgical disposables, doctor and consultant visit charges, dressing charges, ICU and ICCU charges, injection charges, monitoring charges, nursing care charges, oxygen and ventilator charges, operation charges, operation theatre charges, physiotherapy charges, surgeon fees, registration charges, related routine investigations during admission, transfusion and blood processing charges, and equipment charges including infusion pumps and syringe pumps.
In simple words — everything the hospital uses to treat you from the moment you are admitted to the moment you are discharged is covered under the package rate.
What is NOT Included in the Package Rate?
The ECHS SOP is equally clear about what the package does not cover. Items that are not part of the package and can be collected from the patient are —
Telephone charges for patient calls, television charges, toiletries and personal items such as soap, shampoo, toothbrush, toothpaste, sanitary pads, diapers, tissue papers, and hand sanitisers for personal use. Cosmetics and personal items such as razors, beauty creams, powders, deodorants, and talcum powder are also not covered. Attendant food and carry bags for medicines are not covered. Mortuary or cremation charges if a patient expires are also not covered by ECHS.
How Long Does a Package Rate Last?
Different treatments have different package durations —
Specialised super specialty treatment — up to 12 days
Major surgeries — up to 7 days
Laparoscopic surgeries, elective angioplasty, and normal deliveries — up to 3 days
Day care and minor OPD surgeries — 1 day
Any patient who needs to stay beyond the package period requires the hospital to seek online approval for extension from ECHS authorities with full medical justification.
Rules for Multiple Surgeries in One Session
If a patient undergoes more than one surgery in the same operation theatre session, the reimbursement is calculated as follows —
The primary surgery — which is the one with the highest package rate — is reimbursed at 100 percent.
The second surgery in the same session is reimbursed at 50 percent of its package rate.
The third and any subsequent surgeries in the same session are reimbursed at 25 percent each.
For identical surgeries at different anatomical sites such as bilateral procedures in one session — the first is reimbursed at 100 percent and the second at 50 percent.
Rules for Procedures Within the Same Admission
If a procedure is performed during the package period of an earlier procedure — meaning a different procedure on a different date but within the same admission and within the package period — the hospital is reimbursed at 75 percent of the applicable package rate for the subsequent procedure.
Individual steps of a procedure cannot be itemised or charged separately. All steps of a procedure are considered part of the package.
Implants — Separate from Package
Implants such as lenses, stents, meshes, and valves are reimbursable in addition to the package rate as per CGHS ceiling rates. If a patient chooses a higher value implant than what ECHS covers, the patient pays only the difference. Written consent from the patient must be obtained by the hospital.
