Why Do ECHS Claims Get Stuck?

Getting treatment under ECHS is usually smooth. But when it comes to claim processing and reimbursement, many Ex-Servicemen and their families face unexpected delays or rejections. The frustrating part is that most of these problems are avoidable. They happen not because of any fault in the system but because of gaps in documentation, referral conditions, or billing procedures.

Understanding why claims get stuck helps you protect yourself from unnecessary trouble.

Problem 1 — Incomplete or Missing Documents

This is the number one reason for claim delays. ECHS claim processing is completely document-based. If even one important paper is missing, the entire claim can be held up for weeks.

Documents that are commonly missing or forgotten include discharge summary, referral papers, prescriptions written by specialist, investigation reports, hospital bills, and emergency certificates where applicable. Beneficiaries often assume the hospital will manage everything. But when objections are raised during claim verification, it is the beneficiary who faces the delay.

The SOP clearly says that on discharge, the hospital must provide one complete copy of all medical records to the patient free of cost. This includes lab reports, X-rays, CD copies of scans, and the discharge summary. Demand this before leaving the hospital. Do not leave without your complete file.

Problem 2 — Referral Issues

The referral is the official permission document that authorises your treatment at an empanelled hospital. If your referral has expired, is of the wrong type, or does not cover the treatment you received, your claim can face serious objection.

Key referral rules you must know. A standard OPD referral is valid for three months. For cancer, diabetes, hypertension, cardiac, and dialysis patients the referral validity is six months. An OPD referral cannot be used for hospital admission — you need a separate IPD referral for that. Beneficiaries aged 70 years and above can visit empanelled hospitals directly for OPD consultation without any referral. Most claim problems related to referrals happen because beneficiaries did not know these basic rules.

Problem 3 — Billing and Package Disputes

Billing disputes are very common during claim processing. The ECHS follows fixed package rates for different treatments, surgeries, and procedures. If a hospital bills for items that are already included in the package, or adds charges for things that are not admissible, the claim gets objected.

Items that cannot be charged separately and are already part of the package include glucometer strips, ECG electrodes, ventilator circuits, nursing care charges, routine ward facilities like infusion pumps and pulse oximeters, and air beds. Items that are completely non-admissible and cannot be charged to ECHS at all include TV charges, telephone charges, toiletries like soap and toothpaste, cosmetics, and food for attendants. If a hospital includes these in your bill, ECHS will disallow them during claim processing.

Problem 4 — Emergency Treatment Documentation Gaps

Emergency cases are covered under ECHS without any referral. But even in an emergency, proper records are essential for claim processing. The hospital is required to raise an Emergency Intimation Report on the BPA portal within 48 hours of admission. A geo-tagged photograph of the admitted patient must also be attached. If these steps are missed by the hospital, claim complications arise later. As a beneficiary you should ensure that your attendant follows up with the hospital during admission to confirm that emergency documentation is being completed.

What You Should Always Do

Preserve every document from the first day of treatment. Verify referral validity before going to any empanelled hospital. Check your bills at the time of discharge before signing anything. Ask for your complete medical file before leaving the hospital. Keep photocopies of everything at home.

Most ECHS claims that get delayed could have been processed smoothly with just a little more attention to paperwork from the beginning.

Frequently Asked Questions

Q1. Who actually submits the ECHS claim — the beneficiary or the hospital?

In cashless treatment cases, it is the empanelled hospital that submits the claim online through the BPA portal to ECHS. The beneficiary’s role is to provide correct documents during treatment and to preserve copies of all records. In reimbursement cases, the beneficiary submits the claim directly through ECHS channels.

Q2. What is the most common reason ECHS claims get delayed?

Incomplete documentation is the most common reason. Missing discharge summary, expired referral, absent investigation reports, or incomplete hospital bills are the biggest causes of claim delays during verification by ECHS authorities.

Q3. What happens if my referral has expired before treatment is complete?

If your standard referral has expired, treatment continuation may not be covered. You should go back to your Polyclinic and get a fresh referral issued before continuing treatment. Never assume an expired referral will be accepted during claim processing.

Q4. Can emergency treatment claims get rejected?

Yes, even genuine emergency claims can face problems if proper records are missing. The hospital must raise an Emergency Intimation Report within 48 hours and attach a geo-tagged photograph of the patient. If the hospital misses these steps, the claim may face objection even if the emergency itself was genuine.

Q5. What items in a hospital bill does ECHS not pay for?

ECHS does not pay for TV charges, telephone charges, food for attendants, personal toiletries like soap and toothpaste, cosmetics, razors, and other personal convenience items. These are non-admissible charges and cannot be included in ECHS claims.

Q6. Can glucometer strips and ECG electrodes be charged separately?

No. Glucometer strips used for bedside blood sugar testing and ECG electrodes are considered part of the test cost and cannot be billed separately. If a hospital bills these items separately, ECHS will disallow them during claim verification.

Q7. What should I do if the hospital gives me a vague bill without proper details?

Ask for an itemized bill before discharge. The ECHS SOP specifically states that vague billing entries like “consumables kit” without specific details are not acceptable and will be disallowed during claim processing. You have the right to a clear and detailed bill.

Q8. What is the right of ECHS beneficiaries regarding medical records at discharge?

Every ECHS beneficiary has the right to receive one complete copy of all medical records at discharge — including lab reports, X-ray films, CD copies of scans, and the discharge summary — completely free of cost. Do not leave the hospital without collecting your complete file.

Q9. Why do billing disputes happen so often in ECHS claims?

Most billing disputes happen because hospitals sometimes include charges for items that are already part of the fixed package rate. Nursing care charges, equipment charges, ward facilities, and routine consumables used during surgical procedures are all included in the package and cannot be separately billed.

Q10. What is the single most important thing a beneficiary can do to protect their ECHS claim?

Start preserving documents from day one of treatment. Keep the original referral safe, collect your complete discharge file before leaving the hospital, keep photocopies of all bills and reports, and never depend only on the hospital to manage your claim paperwork. Proper documents are your strongest protection in any ECHS claim situation.
