Many veterans and their families do not know this and either end up in a lower category room than they deserve or pay extra for a higher room unnecessarily.
Under ECHS, your ward-entitlement is decided by your rank at the time of retirement. It does not matter what hospitals you are in or which city you are in — the entitlement follows your rank. Rank Decides Your Room
ECHS has three categories of ward entitlement. Every retired defence personnel falls into one of these three categories based on their rank at retirement.
Category 1 — General Ward
This entitlement is for NCOs and below from all three forces. This includes:
Army — Recruit, Sepoy, Naik, Havildar
Indian Air Force — AC, LAC, Corporal, Sergeant
Indian Navy — Sea-II, Sea-I, Leading Seaman, Petty Officer
All personnel of these ranks are entitled to a General Ward during hospital admission.
Category 2 — Semi-Private Ward
This entitlement is for JCOs in the Army and their equivalents in the Navy and Air Force including Honorary ranks. This includes:
Army — Honorary Naib Subedar, Naib Subedar, Subedar, Subedar Major, Honorary Lieutenant, Honorary Captain
Indian Air Force — Junior Warrant Officer, Warrant Officer, Master Warrant Officer, Honorary Flying Officer, Honorary Flight Lieutenant
Indian Navy — Honorary Chief Petty Officer, Chief Petty Officer, Master Chief Petty Officer II, Master Chief Petty Officer I, Honorary Sub Lieutenant, Honorary Lieutenant
All personnel of these ranks are entitled to a Semi-Private Ward.
Category 3 — Private Ward
This entitlement is for all Officers of the Army, Navy, and Air Force. This includes:
Army — Cadet, Lieutenant, Captain, Major, Lieutenant Colonel, Colonel, Brigadier, Major General, Lieutenant General, General
Indian Air Force — Flying Officer, Flight Lieutenant, Squadron Leader, Wing Commander, Group Captain, Air Commodore, Air Vice Marshal, Air Marshal
Indian Navy — Sub Lieutenant, Lieutenant, Lieutenant Commander, Commander, Captain, Commodore, Rear Admiral, Vice Admiral, Admiral
All officers of these ranks are entitled to a Private Ward.
What is Included in Your Ward Entitlement?
Your room entitlement is not just the bed. The ECHS SOP April 2026 clearly states that room rent as per entitlement includes — charges for the bed, linen, patient diet and meals, nursing charges, and charges for electricity and water. No separate charges can be levied on the ECHS patient for any of these items.
What if the Entitled Ward is Not Available?
If the ward you are entitled to is not available at the time of admission, you may be temporarily accommodated in the next higher category ward until your entitled category becomes available. This is allowed but only as a temporary arrangement.
However if a hospital does not have your entitled ward category at all, the hospital can only bill ECHS at your entitled category rate — even if you were treated in a higher category ward. You do not pay the difference in such cases.
What if You Want a Higher Category Room?
If an ECHS patient personally requests a higher category room than what they are entitled to, it can be provided subject to availability — but on payment basis. The patient pays only the difference between the entitled category rate and the higher category rate. The hospital must obtain written consent from the patient before providing the upgraded room.

Not knowing your rank-based ward entitlement and accepting a lower category room without questioning it.
Paying for a higher room without knowing that if the hospital does not have your entitled category, they cannot charge you extra.
